IN CONSIDERATION of being permitted to participate in any of the activities of Orion Dog Training, the undersigned, for himself/herself and for all personal representatives, heirs, assigns and next of kin:
1. HEREBY RELEASES, WAIVES, DISCHARGES AND COVENANTS NOT TO SUE Orion Dog Training/ Djoeke van der Velde, Furrtropolis, Petfood Express and/or any of its members, employees, officers, owners, agents, promoters, operators, representatives and/or other participants for any loss or damage, and any claim or demands therefore, on account of injury to the person or property, or resulting in death, of the undersigned, arising out of or in any way related to Orion Dog Training activities, whether caused by the negligence of the Orion Dog Training/ Djoeke van der Velde, Furrtropolis and Petfood Express/or any of its members, employees, officers, owners, agents, promoters, operators, representatives and/or other participants otherwise.
2. HEREBY EXPRESSLY ASSUMES FULL RESPONSIBILITY FOR ANY RISK OF BODILY INJURY, PROPERTY DAMAGE OR DEATH arising out of or related to Orion Dog Training activities, whether caused by the negligence of the Orion Dog Training/ Djoeke van der Velde Furrtropolis and Petfood Express/or any of its members, employees, officers, owners, agents, promoters, operators, representatives and/or other participants otherwise;
3. HEREBY ACKNOWLEDGES THAT PARTICIPATION IN Orion Dog's ACTIVITIES MAY BE DANGEROUS and involves the risk of bodily injury, property damage and/or death;
4. HEREBY ACKNOWLEDGES THAT Orion Dog Training ACTIVITIES AS USED IN THIS AGREEMENT includes all Orion dog training functions and activities without restriction of any kind, including, but not limited to, dog training classes, off leash puppy socials, off leash puppy classes, private one on one sessions onsite and off site.
5. A LEGAL GUARDIAN SIGNING BELOW ON BEHALF OF A MINOR CHILD AGREES TO INDEMNIFY AND HOLD HARMLESS the Orion Dog Training/ Djoeke van der Velde , Furrtropolis and Petfood Express/or any of its members, employees, officers, owners, agents, promoters, operators, representatives and/or other participants from any claim for injury or damage to such child.
Photo Release form: I hereby grant Orion Dog Training, Dieuwke van der Velde and its representatives permission to use and or publish photographs and or videos of my pet and or myself in print and or electronically . I understand and agree that these materials will become property of Orion Dog Training and will not be returned. I hereby authorize Orion Dog Training to edit, alter, exhibit, publish or distribute the photograph or video for purposes of publicizing their programs or for any lawful purpose. In addition I waive my rights to any compensation arising or related to the use of the photographs or videos.
